Learned from a Major Accident-Ammonia Plant Syngas compressor oil tank
explosion 🔥
ACCIDENT DESCRIPTION 💥
On the 24th of April 2017, at 2:43 am, a trip occurred in an ammonia plant due
to an external power supply fluctuation. The dip lasted 0.4 seconds. This
fluctuation caused an automatic shutdown of the plant, and a normal shutdown
operation was performed. Flammable content flared off. After stabilizing the
process, the production shift was preparing the plant for restart. Everything
seemed normal, restart procedures were followed, but at 03:15, an explosion
occurred, followed by an intense oil fire in the compressor hall. Two operators
were present and evacuated without physical injuries. Internal and external
fire services arrived shortly after, and 45 minutes later, the fire was
extinguished. They could see that the main syngas oil tank (lubricant oil) had
"torn," and the fire could be localized to this tank. Immediate
consequences on people were limited to psychological impact (one employee on
long sick leave). In terms of the environment, there was a limited oil release
into the fjord that was rapidly addressed. Extensive damage was observed in the
electrical/instrumentation systems and in the compressor hall.
SUBSTANCE INVOLVED DESCRIPTION
The vapor phase of the oil tank normally contains a mixture of lube oil vapor
and syngas released by the seal oil circuit. Gas analysis has been conducted on
a similar system, and it was observed that the Hydrogen concentration ranged
between 0.1 to 2%. The quantity is difficult to estimate.
CAUSES DESCRIPTION
The root cause lay in an uncontrolled Management Of Change (MOC) affecting the
air separation unit trip system that allowed enriched air to be sent into the
nitrogen grid. The nitrogen grid was supposed to be a safety barrier in the
syngas compressor oil tank explosion scenario. A low suction pressure trip had
been removed some years back (2011) but was still mentioned in both P&ID
and standard operating procedures. Contributing factors included a lack of Process
Hazard Analysis that generated inadequate standard operating procedures and
inadequate training. There was also a lack of a proper alarm management program
and a poor human-machine interface.
EMERGENCY RESPONSE 🚒
Emergency response teams acted swiftly and effectively. The on-site water mist
system played a crucial role, although a minor issue with foam leakage arose
due to an open door.
